Key Clinical Message Systemic to pulmonary venous collaterals (SPVCs) are common findings in patients with Fontan and may be responsible for desaturation in this group of patients. In selected cases, percutaneous closure of SPVCs with coils or vascular plugs could be offered as a safe and effective treatment modality for these patients.

We report the case of a 3-year-old boy who underwent definitive conversion to the Fontan circulation after total cavopulmonary shunt using a total extracardiac right heart bypass. This simple, safe and reproducible procedure is an alternative to Fontan or total cavopulmonary connection procedure.

Total cavopulmonary connection--early results.
Presently total cavopulmonary connection is used as an alternative in complex congenital disorders requiring atriopulmonary connection. From January 1989 till November, 1992, a total of 18 cases underwent total cavopulmonary connection. There were 8 males and 10 females. The mean age at operation was 92 months (range 5 to 504 months).
