Total gastrectomy is indicated principally for Zollinger-Ellison syndrome and for potentially curable gastric cancer. The diagnosis of cancer should be verified by biopsy before the resection is performed, and the margins of resection should usually be examined by frozen section.

A 2-day-old, female infant underwent total gastrectomy for spontaneous gastric gangrene and perforation. She developed the following complications: growth re tardation, vitamin B12 deficiency, post-gastrectomy dumping syndrome, and oc casional bile regurgitation. Nutritional management is described.

AbstractTotal gastrectomy has more frequent and serious postoperative complications than subtotal gastrectomy. An esophago‐gastric stoma with a side‐to‐side anastomosis permits a near‐total gastrectomy to be undertaken with virtually the same results as a total gastrectomy.

CREDIT for the first successful total gastrectomy goes to Schlatter, 1 who in 1897 reported his case in which gastrointestinal continuity was reestablished by means of an esophagojejunal anastomosis. It was just one year later that Brigham 2 accomplished the operation successfully by employing an end to end esophagoduodenostomy.
