Chronic total occlusions (CTOs) remain the most technically challenging and time-consuming subset of coronary artery lesions to treat with percutaneous intervention (PCI). Particular difficulties may be encountered in crossing and dilating the lesion, whilst the rates of restenosis are higher than other lesions even after successful PCI.

When crossing into the distal true lumen fails during chronic total occlusion (CTO) percutaneous coronary intervention (PCI), subintimal plaque modification (SPM) is often performed to restore antegrade flow and facilitate subsequent lesion ...

Total coronary artery occlusion.
Cardiovascular clinics, 1988Although once believed to be unsuitable for coronary angioplasty, patients with subacute and chronic total occlusions have now emerged as important candidates for this procedure. They account for between 10% and 15% of current angioplasties. With careful attention to patient selection factors (occlusion duration, length, morphology, associated thrombus)
