Evaluation of Transanal Endoscopic Surgery for Rectal Tumors.
The Japanese Journal of Gastroenterological Surgery, 2001 早期直腸癌の局所治療は癌浸潤度, 腫瘍の局在部位・大きさによって治療方法は選択される. なかでも大きな腺腫, 粘膜癌, 一部のsm癌に対する低侵襲手術方法として管腔内視鏡手術が要求される. そこで, 早期直腸癌に対するTEMの適応についてEMRと比較検討し, さらにTEMの適応拡大のために手術手技の工夫を加えた.腫瘍の大きさでは腫瘍径が15mm以上の病変はEMRで水平方向の断端陽性となることがあり, TEMの手術適応と考えられた. また, 腫瘍の局在ではBuess式直腸鏡を使用するために肛門縁より5～20cmの直腸腫瘍がTEMの良い適応と考えられた.
