Orofacial involvement secondary to trapezius muscle trauma

The case reported in this article demonstrates an unusual referral mechanism for signs and symptoms of the face and the oral cavity emanating from a traumatized trapezius muscle. A theory for this referral, based on central connections of nerves V, VII, IX, X, and XI, is proposed.

In 10 healthy subjects stimulation of the accessory nerve in the posterior triangle of the neck always elicited action potentials in all 3 parts of the trapezius muscle. Of 8 patients with a lesion of the accessory nerve 3 had no evoked potentials, while 5 showed evoked potentials in the 3 parts of the muscle. 7 of these patients had voluntary activity

Four cases of paralysis of the trapezius muscle occurring during the last 15 years are reported. The characteristics of this lesion, which is usually of iatrogenic origin, are outlined. Among the alternatives in treatment, Eden's operation is the method of choice. Major handicaps are usually corrected from both an esthetic and functional point of view.
