An unusual case of “unipolar” pacing and myopotential oversensing leading to an inappropriate ICD shock in a patient with an implanted defibrillator is reported. The reasons for unipolar behavior in a system using a committed bipolar device are discussed.

Unipolar depression is characterized by a combination of two types of symptoms: neurovegetative and emotional-cognitive. Neurovegetative symptoms are those symptoms that are directly related to the body (e.g., insomnia/hypersomnia, dysregulated eating, fatigue, and decreased energy).
