Noninvasive Identification of the Unstable Carotid Plaque

Annals of Vascular Surgery, 2006
Intraplaque hemorrhage, enlarging lipid cores, and their proximity to the flow lumen are important determinants of carotid plaque rupture and neurological complications. We developed an image-analysis method for B-mode ultrasound, pixel distribution analysis (PDA), for preprocedural identification of these high-risk features in carotid plaques.

Floating Thrombus as a Marker of Unstable Atheromatous Carotid Plaque

Annals of Vascular Surgery, 2011
Floating thrombus in a carotid artery is an uncommon pathology with a high risk of embolism. We present three patients diagnosed with acute stroke, with a floating thrombus complicating an atheromatous plaque, who were treated with anticoagulants and statins.

Echomorphologic and histopathologic characteristics of unstable carotid plaques.

AJNR. American journal of neuroradiology, 2001
Our hypothesis was that the carotid plaques associated with retinal and cerebrovascular symptomatology and asymptomatic presentation may be differ from each other. The aim of this study was to identify the sonographic and histopathologic characteristics of plaques that corresponded to these three clinical manifestations. The echo process involved duplex

Circulating Lipoprotein-associated Phospholipase A2 in High-grade Carotid Stenosis: A New Biomarker for Predicting Unstable Plaque [PDF]

open access: yesEuropean Journal of Vascular and Endovascular Surgery, 2012
ObjectiveTo test plasma levels of lipoprotein-associated phospholipase A2 (Lp-PLA2) in patients with high-grade carotid stenosis according to plaque histology.MethodsThis cross-sectional single-centre study included patients with ≥70% North American ...

Legumain and cathepsin-L expression in human unstable carotid plaque

Atherosclerosis, 2010
The cysteine protease, legumain, is thought to have a role in the processing and activation of proteases such as cathepsin-L, which have been implicated in plaque rupture. This study aimed to determine: if legumain activity is up-regulated in unstable areas of plaque; the effect of legumain over-expression on the activity of cathepsin-L and the effect ...

Carotid plaque composition in stable and unstable coronary artery disease

American Heart Journal, 2005
Several pieces of evidence suggest that formation of complex atheromatous plaques may be influenced not only by local but also by systemic factors.Twenty-five patients (16 men/9 women, age 63 +/- 10 years) with stable coronary artery disease (sCAD) and 61 (41 men/20 women, age 66 +/- 16 years) with acute coronary syndromes (ACSs) underwent carotid ...

Efficacy of Carotid Endarterectomy for Mild (<50%) Symptomatic Carotid Stenosis with Unstable Plaque

World Neurosurgery, 2019
Carotid endarterectomy (CEA) is known to reduce stroke risk in patients with symptomatic, moderate to severe carotid stenosis but has no apparent impact in patients with symptomatic, mild (less than 50%) carotid stenosis. However, recent development of noninvasive imaging modalities has shown that a certain subgroup of patients are at high risk for ...

Unstable Carotid Artery Plaque Evaluation by Ultrasound

2017
Although evaluation of carotid stenosis has been traditionally focused on degree of stenosis, there is a growing evidence about the importance of the evaluation of morphological features of carotid plaques. Unstable plaques can lead to stroke regardless of the degree of stenosis they cause.

[Unstable carotid plaque: biochemical and cellular marker of vulnerability].

Italian heart journal. Supplement : official journal of the Italian Federation of Cardiology, 2003
Stroke is the second most common cause of death in developed countries. Carotid plaque disruption and distal embolization of atheromatous debris are the most common pathogenic mechanisms for cerebral ischemia from carotid atherosclerotic disease. Morphologic composition of the atherosclerotic plaque, rather than the stenotic severity, appears to be ...

Progress in the identification of unstable carotid artery plaque

Current Opinion in Neurology
Purpose of review Ischemic stroke remains a leading cause of death and disability worldwide, with carotid atherosclerosis as a major underlying mechanism. For decades, treatment decisions were based primarily on luminal stenosis, overlooking the biological complexity of plaque instability. This review summarizes
