Visual Internal Urethrotomy

Summary— Seventy‐one patients with urethral strictures were treated by internal urethrotomy with the Storz‐Sachse optical urethrotome. A good result was obtained in 70% of patients, whether a catheter had been left in for 7 or for 28 days.

Transurethral urethrotomy under vision with the Sachse urethrotome is a new surgical procedure for the treatment of urethral strictures. The chief advantage of the urethrotome is the fact that the surgeon can cut strictures selectively and accurately under clear vision.

Cold knife optical urethrotomy

Cold knife optical urethrotomy with the Sachse urethrotome was used in 44 patients with urethral strictures. Appraisal if the results, after a mean follow-up of fourteen months, showed 77.3 per cent success rate regarding urine flow rates and 81.8 per cent regarding subjective response of the patient.

Pelvic urethrotomies in bulls

Pelvic urethrotomies were attempted on 32 bulls (12 cattle and 20 buffaloes). Infra-anal urethrotomy was carried out under a tranquilliser, posterior epidural and supra-ischial blocks. Prebulbomuscular urethrotomy was performed under a tranquilliser, posterior epidural and pelvic plexus blocks.

Optical Urethrotomy—Experience in Oman

Summary— Over a 4‐year period 170 men with urethral strictures were treated with optical urethrotomy; 80% required no further treatment during the 2‐year follow‐up; 2 patients with urethral fistulae associated with stricture were also treated successfully.

Optical urethrotomy using topical anesthesia

A series of 28 patients with urethral strictures less than 2 cm length underwent 32 visual optical internal urethrotomy under local urethral anesthesia with lidocaine. The procedure was completed successfully in 26 of 28 (92.9%) patients. Among these 25 of 26 (96%) patients reported mild pain.

[Internal urethrotomy].

Internal urethrotomy is the first treatment for all kinds of urethral strictures except meatal stenosis and complete urethral obstruction. After urethrotomy a urethral catheter is left indwelling for up to 3 days if the urine is already sterile. In the case of infection it remains in place for 2 weeks.
