症例は68歳女性.繰り返すと下痢と意識障害の既往あり.異常行動にて入院し,代謝性アシドーシスと高アンモニア血症に伴う代謝性脳症と診断した.肝,腸管には異常なく,骨シンチグラフィーにて結腸全体に異常集積を認め,膀胱造影にて膀胱回腸瘻と診断した.脳症発現にアシドーシスに加え,尿中日和見感染菌によるアンモニア産生と腸管からの吸収の寄与が考えられた.
