Vascularized bone transfers are the ideal bone graft since they provide their own blood supply and all necessary osteoinductive, osteoconductive, and osteoprogenitor elements. These attributes allow them to be used in compromised recipient sites. When used as onlay grafts to augment skeletal contour they provide the best opportunity to maintain volume.

Fixation of the Vascularized Bone Graft in Mandibular Reconstruction

One hundred and forty vascularized bone grafts were used for mandibular reconstruction in 135 patients. Most followed surgical ablation of squamous carcinoma recurring (or persisting) after irradiation. This paper analyzes the fixation techniques and their effects on bony union. Of 140 vascularized bone grafts, 132 survived and were reviewed.

The Current Status of Free Vascularized Bone Grafts

In the last decade, free vascularized bone transfer has become an established technique, with success rates above 90 per cent reported in most series. These operations have special application for those situations in which the bone defect is large, the recipient bed is poorly vascularized, and there is an associated soft tissue deficiency.
