Permanent or transient reduction of the blood flow to the brain is often responsible for dizziness and occasionally sensorineural hearing loss. So far the diagnosis of this condition has been made by angiography, either carotid or vertebral arteriography, but there is a justified reluctance of subjecting unselected patients to this invasive procedure.

After critically reviewing the last 50 years' literature pertaining to vertebrobasilar insufficiency, we reached the following conclusions: One can seldom accurately localize vascular pathologic lesions in the posterior circulation by clinical examination alone. The symptoms of vertebrobasilar insufficiency have multiple causes.

✓ Extracranial vertebrobasilar artery thrombo-occlusive disease may cause repetitive transient ischemic episodes and, less frequently, brain-stem or cerebellar infarction. This report describes 40 patients who experienced repetitive vertebrobasilar ischemic symptoms despite maximal medical therapy.

Vertigo is the most common symptom of transient ischemia within the vertebrobasilar system. Episodes typically come on abruptly and last minutes. Although there are usually associated symptoms, isolated attacks of vertigo can occur. Vertigo is also a common symptom with brain stem and cerebellar strokes.

Four cases of vertebrobasilar insufficiency are reported. Case 1 was a 38-year-old man who felt a sudden onset of dizziness when he turned his head to the back. An abnormal positional nystagmus was observed when he rotated his head to the left. A magnetic resonance angiogram (MRA) demonstrated total occlusion of the left vertebral artery (VA).
