縦隔鏡検査は, 気管周囲のリンパ節病変の診断に極めて有用であり, 多くの施設で行われている.反面, 縦隔という狭い空間での操作であるため, 従来の直視型縦隔鏡では, 術者にしか観察できないこと, 術者の操作姿勢の不自由さ, 鉗子類挿入による観察視野の狭小化など, 改善すべき点が多く, 敬遠する向きも多いのが現状である.そこでわれわれは, Video-Assisted Thoracic Surgeryの技術と器具を縦隔鏡検査に応用する方法を考察した.縦隔鏡筒右端の開放部分に沿って, 直径5rnrnの内視鏡を挿入し, ビデオモニターの映像を観察しながら, 内視鏡下手術に用いる鉗子類を使用して, リンパ節を周囲の脂肪組織や結合織から剥離, 採取する.この方法により, 術者と助手が同時に観察可能となり ...
