Transverse Volar Skin Laceration of the Finger: A Sign of Volar Plate Injury

Transverse tears of the volar skin of a finger with a hyper-extension injury is associated with frequent volar plate tears or avulsions. Recognition is essential for surgical exploration of the volar plate and supporting joint structures and, if injured, surgical repair should be carried out at the time of skin suturing.

With the advent of locking volar plates, many distal radius fractures, including those with comminution, are able to be adequately stabilized from a volar approach only. However, some fractures require supplemental fixation to adequately stabilize and prevent escape of the dorsal-ulnar fragment, and with it, subluxation of the carpus.

A significant number of distal radius fractures can be appropriately managed with a volar locking plate alone. However, fractures with a radial column/styloid fragment can prove difficult to capture with a standard volar locking plate, as the primary or secondary fracture lines would be parallel to the distal screws of most volar plates.

Abstract Irreversible volar plate contracture (bent finger) can develop late (up to 3 months). Follow up all these injuries at least once. Thereafter, tell the patient to return if the finger starts to bend and will not straighten completely. Then refer to hand surgeon or therapist.
