High‐grade non–muscle‐invasive bladder cancer (NMIBC) with lymphovascular invasion is rare in young adults. We report a 27‐year‐old male, a heavy hookah smoker, who presented with painless gross hematuria. Cystoscopy revealed a large papillary bladder tumor.
