Abdominal computed tomography scan documented a right renal mass. A right nephrectomy was performed. The pathological report documents xanthogranulomatous pyelonephritis.

Amyloidosis, hitherto unreported in association with this lesion, was found in the liver in one case. The need for an accurate preoperative diagnosis and long-term follow up are discussed.
