The authors report on 21 cases of "primary" xanthoma of bone. Twenty of the patients were older than 20 years old. The male-female ratio was 2:1. The presenting symptom was pain in 13 patients and neurologic symptoms in 2; in 6 patients, the lesion was an incidental finding.

This communication reports an unusual radiological finding in a patient with familial hyperlipaemia. Some aspects of the literature pertaining to this condition are discussed. A 38-year-old white female presented with angina-like pains of 13 years' duration. There was a strong family history of myocardial infarctions.

Primary xanthoma in foot bones is an extremely rare event. In an individual without a family history of hyperlipemia, it is even a rarer finding. One case of a xanthoma of a calcaneus is presented in an otherwise asymptomatic 19-year-old normal lipidemic female. The previous literature is reviewed.

Xanthoma is a lesion containing abundant foamy histiocytes most commonly occurring in superficial soft tissues such as skin, subcutis, or tendon sheaths. The involvement of deep skeletal structures, however, is rare and has only been infrequently reported in the English literature. Most xanthomas occur in patients with hyperlipidemic disorders.
