The Zenker’s diverticulum is an out pouching of the hypopharynx arising between the fibers of the cricopharyngeus inferiorly and the inferior constrictor superiorly. This region of herniation is known as Killian’s triangle. Patients often present with a longstanding history of gradually increasing dysphagia of both solids and liquids.

Pharyngoesophageal (Zenker’s) Diverticulum
Surgical Clinics of North America, 1983The Mayo Clinic surgeons have the largest experience in the world in dealing with pharyngoesophageal diverticulum. This experience has now grown to 888 patients treated between the years 1944 and 1978. In that period, their approach has undergone changes. The reasons behind these changes and their results are discussed.

Treatment of Zenker's diverticulum
Gastrointestinal Endoscopy, 2001Although Ludlow1 in 1769 gave the first description of a pharyngo-esophageal diverticulum, it is named after Zenker who gave a precise pathologic description more than one century later2. Zenker’s diverticulum (ZD) is the commonest type of diverticulum in the upper gastro-esophageal tract.

There is now strong evidence that acquired Zenker’s diverticulum arises in most cases secondary to a poorly compliant, but normally relaxing, UES which cannot fully distend during the process of sphincter opening. This gives rise to increased hypopharyngeal intrabolus pressure during the phase of trans-sphincteric bolus flow; pressure which is imparted

The first case of posterior pharyngooesophageal diverticulum was published in 1764 by Ludlow. Zenker's name has been attributed to the diverticulum since his description of a series of patients in 1878. The aetiology and pathogenesis of Zenker's diverticulum are not well understood.

The Zenker posterior pharyngoesophageal diverticulum results from the posterior herniation of the hypopharyngeal intima near its junction with esophagus. To its occurrence contribute more factors exerted at the level of Leimer's triangle. It has a slow progressive evolution and may thus vary in size from a beam to an apple or even the head of a foetus,
