Abstract

Acute Type A aortic dissection is a life-threatening surgical emergency associated with significant morbidity and mortality. Patients often present with concomitant aortic valve (AV) insufficiency either due to underlying aortopathy or valve disease or directly related to the acute dissection. Regardless of etiology, the surgeon has to make complex decisions on whether to intervene on the aortic root and valve with many different permutations of AV repair available in the surgical armamentarium. An understanding of the anatomy, physiology, and mechanism of aortic insufficiency is the first step towards a systematic assessment of every case and ultimate decision on whether to preserve and repair the valve and how to best do so.
