Zusammenfassung

Perioperativ auftretende Gerinnungsstörungen und die konsekutive Transfusion von allogenen Blutprodukten stellen einen unabhängigen Prädiktor für kardiale und nichtkardiale Ereignisse dar und wurden mit erhöhter perioperativer Letalität assoziiert. Schnellstmögliche Diagnose und effektive, zielgerichtete Therapie der zugrunde liegenden Gerinnungsstörung sind demnach von hoher klinischer Relevanz in der Intensivmedizin. Neben den im Zentrallabor durchgeführten klassischen stehen zunehmend verbreitet sog. Point-of-care- (POC-) Methoden zur bettseitigen Diagnostik und zum Therapiemonitoring von Gerinnungsstörungen zur Verfügung. Deren Spektrum reicht vom einfachen Streifentest bis zu komplexen Verfahren, deren Bedienung und Ergebnisinterpretation eingewiesenen und trainierten Untersuchern vorbehalten ist.
