A 59-year-old man presented acutely with generalized abdominal pain and tenderness. He was cardiovascularly stable but had an elevated white cell count of 27610 l and a C-reactive protein of 250 mg l. His biochemistry showed a degree of metabolic acidosis and dehydration, and he had been previously fit and well.
