The aim of this study was to examine the effect of these changes in relation to outcome following abdominal aortic aneurysm (AAA) repair; a vascular "indicator" operation as defined by the U.K. Royal Colleges.

TSP4 is upregulated in human and experimental AAA. TSP4 knockdown accelerated AAA formation and exacerbated both vascular inflammation and remodelling, emphasizing that TSP4 upregulation may represent a protective response against AAA. Our findings suggest that preserving TSP4 could be a useful strategy in the clinical management of AAA.

Recurrent Suprarenal Abdominal Aortic Aneurysm(AAA) after Repair Infrarenal AAA.
72歳女性の腎動脈下腹部大動脈瘤術後5年目に腎動脈上, 横隔膜下に及ぶ腹部大動脈瘤を認め, 手術を施行した. Stoney の方法に準じ, 開胸腹膜外到達法を用い, 大動脈に到達した. 補助手段として, 下大静脈脱血, 大腿動脈送血の Biopump, 人工肺を用いたV-Aバイパスを造設し, 大動脈遮断中, 腹腔動脈, 上腸間膜動脈, 左右腎動脈に循環カテーテルを用いて灌流を行った. 術直後は軽度の腎機能障害, 肝機能障害が出現したが, 第20病日, 大動脈造影で再建分枝の開存を確認し, 退院した. 本例のような虚血性心疾患を合併した再手術例では, V-Aバイパス補助下の手術が臓器保護, 循環動態の安定に推奨されると思われた.
