術前に診断困難であった,右固有乳腺分泌癌および同側副乳癌を併存した症例を経験したので報告する.症例は78歳,女性.右腋窩腫瘤を主訴に来院した.右腋窩上腕寄りに径1.5cm大の弾性硬可動性良好な皮下腫瘤を触知した.両側乳房には明らかな腫瘤を触知しなかった.術前画像診断では右乳癌が強く疑われたが,穿刺吸引細胞診で確定診断に至らなかった.右腋窩腫瘤は穿刺吸引細胞診で乳癌疑いであり右乳癌の腋窩リンパ節転移と考え2008年1月右乳房切除術・腋窩リンパ節郭清および右腋窩腫瘤摘出術を施行した.最終病理組織診断は,右乳腺は分泌癌,右腋窩は硬癌で,腋窩腫瘍浸潤部の辺縁に正常乳腺組織や乳管内病変が認められたため,副乳原発癌と診断した.現在Exemestane内服し経過観察中である.
