A Case of Acrodermatitis Continua of Hallopeau.

37歳の男性。両手指·手掌, 両足趾および足底に限局する鱗屑, 膿疱, 痂皮を主訴として来院した。膿疱の細菌培養·真菌培養では有意な菌は検出されず, 病理組織学的には表皮内の好中球を主体とした海綿状膿疱を認めた。以上の結果より本症例を稽留性肢端皮膚炎と診断し, ステロイド外用およびPUVA療法を開始したが, 経過良好である。稽留性肢端皮膚炎は膿疱性乾癬の1亜型といわれている比較的稀な疾患であり, 本邦では我々の調べ得たかぎり本症例を含め36例の報告にとどまっている。治療法としてはステロイド以外にも, 外国例も含めエトレチナートやシクロスポリンの有効例の報告があるが, 近年活性化ビタミンD3外用剤であるcalcipotriolが有効との報告もあり, 今後の症例の蓄積が待たれるところである ...
