近年,肺癌の中で腺癌は増加している.肺癌の術前診断において,細胞診は生検とともに最も簡便で信頼できる有用な手段である.喀痰中に見られる腺癌細胞は,大きく,円形または多角形で,乳頭状あるいは球形のクラスターで,あるいは孤立性に認められる.細胞質は泡沫状であるか,ライトグリーンに淡く染色される.多量の粘液が存在すると,核を一方に圧排する.腺癌細胞の核は大きく,核のクロマチンは細顆粒状である.核小体は1個で大型,円形で,核の中央に認められる.核縁は肥厚している.気管支擦過標本や経皮的肺生検で得られる標本は,喀痰に比べて出現する腺癌細胞が多く,乳頭状のクラスターを形成するか,シート状に配列する.核のクロマチンは微細顆粒状で均等に分布し,増量している.核縁の肥厚は見られない ...
