Juvenile gouty arthritis is a rare but distinct entity seen in Childhood and adolescence and results from hyperuricemia associated with mild renal insufficiency. The hyperuricemia is much greater than would be expected from the degree of renal insufficiency. There is no family history of inherited renal disease or gout.

• Two patients had onset of juvenile gouty arthritis at ages 16 and 1½ years, respectively. Both had mild renal insufficiency, with creatinine clearances of 46 and 54 mL/min/1.73 sq m, respectively. Their presenting hyperuricemia (13.8 and 11 mg/dL, respectively) was out of proportion to the degree of renal insufficiency.

The authors describe an atypical case of idiopathic gouty arthritis in a 24-year old man. Idiopathic gout is usually found in persons between the ages of 30 and 50 years, and it is unusual to find clinical and roentgenographic abnormalities in a person as young as 24 years of age.
