A CASE OF SUBCAPSULAR BILOMA OF THE LIVER
Nihon Rinsho Geka Gakkai Zasshi (Journal of Japan Surgical Association), 2005 症例は83歳,男性.発熱,上腹部痛,嘔気および黄疸のため入院した.血液検査にて,総ビリルビン値7.9mg/dl,血清アミラーゼ値1,096IU/dlと高値を示し,腹部超音波検査,腹部造影CT,腹部MRIにて,左右肝内胆管の拡張および肝左葉表面の液体貯留像を認めた.経皮的穿刺ドレナージにて胆汁を認め,急性膵炎,閉塞性黄疸を併発したbilomaの診断となった. ERCPにて胆嚢管内に2.0×1.0cm大の結石を認め,総胆管内に明らかな結石は認めなかった.これらの所見から,総胆管に嵌頓した結石により胆道内圧が上昇し,肝内胆管が破綻したためbilomaを引き起こし,その後,結石は十二指腸へ排泄されたものと推測された.総胆管への結石嵌頓により肝被膜下bilomaを形成した稀な症例を経験したので報告した.
