Lost in Transcription: Frequency of Inaccurate Manually Documented Point‐of‐Care Blood Glucose and Ketone Measures in Hospital

Manual transcription of point‐of‐care blood glucose and ketone results into hospital charts was frequently inaccurate compared with automated networked glucose monitoring records. In a two‐month audit of 250 admissions, missing and incorrect entries were common, with many discrepancies clinically significant.
