Synovial Osteochondromatosis in Hip Bursa: A Case Report

滑膜骨軟骨腫症は滑膜の未分化細胞が化生して葡萄の房状に軟骨を生じるものである.過半数が膝関節に発症し,股関節に出現する頻度は約10%である.我々は股関節前面の滑液包に巨大な滑膜骨軟骨腫症を生じた1例を経験したので報告する.症例は73歳男性.約30年前に外傷歴があり,以後同部に軟部腫瘍の自覚あるも症状なく放置していた.H17.5月頃より左股関節の伸展にて大腿前面部に疼痛出現した.CT,MRIにて左股関節前面の滑液包内に多数の結節を認めた.歩行に支障が生じるようになったため,手術を行った.巨大化した滑液包により大腿動静脈は内側に圧排され,大腿神経は前方に圧排され扁平化していた.摘出された滑液包内には径3mm~15mm程度の軟骨遊離体を多数認めた.術後に大腿外側に知覚低下を認めたが,症状は3ヶ月で軽快した.

Synovial Osteochondromatosis in the Subacromial Bursa. A Case Report.

We experinced a rare case of osteochondromatosis in the subacromial bursa. The patient was a 65-year-old man who complained of left shoulder pain, touchable mass lesion and slightly limited of range of shoulder motion. Roentgenograms and arthrography showed several calcified or ossified mass lesions in the extraarticular region.
