横隔膜原発腫瘍は, まれな疾患である.我々は, CA19-9が嚢腫内容液, 末梢血液ともに高値を呈した横隔膜中皮嚢腫を経験したので報告する.症例は72歳男性で, 平成3年2月より右側腹部痛を訴えるようになった.CTにて右上腹部の cysticmass を認め経過観察していたが, 径の増大を認め精査入院となった.入院時現症, 末梢血液像, 一般生化学, 尿所見に異常はなかった.末梢血液中の腫瘍マーカ一はCA19-92.800 U/ml, SPAN-1645 U/ml, CA-50619 U/mlと 高値を呈した.腹部超音波像, CT像, MRI像にて, 右側前外側胸壁に接し, 横隔膜より尾側で胸腔外, 肝外に10×8cm大のcysticmassを認めた.腹腔鏡にて横隔膜由来の嚢腫と確診し ...

A Case of Obstructive Jaundice due to Bile Duct Tumor Thrombus of Hepatocellular Carcinoma Diagnosed by Peroral Cholangioscopy

Abstract While hepatocellular carcinoma (HCC) often invades the portal or hepatic vein to form tumor thrombus, tumor thrombus in the bile duct is rare. In such cases, differentiation from intrahepatic cholangiocarcinoma is difficult, and the tumor often appears as a smooth, yellowish‐white, polypoid mass within the bile duct lumen.

Carbohydrate antigen 19–9 (CA19-9) levels are associated with glycemic control, insulin resistance, and chronic complications in patients with type 2 diabetes (T2D). Women generally show higher CA19-9 levels despite a greater T2D prevalence in men.
