A Case of Cecal Voluvulus with Parkinson's Disease.

症例は79歳の男性で,パーキンソン病があり,抗パーキンソン病薬を内服していた.日常生活は介助により起立が可能だが,ほとんど臥床の状態であった.主訴は心窩部痛,腹部膨満で近医よりイレウスの診断で紹介となった.腹部に巨大なガス像を認め, S状結腸軸捻転症を疑い緊急内視鏡検査を行ったが,上行結腸の肝彎曲部まで正常であった.腸閉塞の診断で経過観察していたが,改善が見られず手術を行ったところ,盲腸は反時計方向に180度回転し,さらに頭側に180度屈曲して盲腸が横行結腸と癒着して著明に拡張していたため,盲腸軸捻転症と診断した.自験例はその誘因としてパーキンソン病と長期臥床による大腸のatonyが深く関与していると考えられた.
