脳梗塞の予防の基本は,各個人が有する脳梗塞に対する危険因子を明らかにし,管理可能なものを管理することである.その多くは動脈硬化の進展を阻止することにつながる.危険因子には高血圧,糖尿病,心疾患,肥満,アルコール多飲,喫煙,脂質異常,年齢,性,経口避妊薬,高ヘマトクリット値,高ヘモグロビン値,高フィブリノーゲン血症,抗リン腸質抗体,薬物などがある.このうち,高血圧の管理が最も重要で,特に集団管理の重要性が注目されている。多くの危険因子の管理は生活様式の工夫,食事面の調整,薬物療法の組み合わせで行われる.抗血小板薬は脳梗塞の一次予防には有効ではない.しかし,抗血小板薬は脳梗塞の再発を,非使用例に比較して約25%減少させる.心原性脳塞栓の基礎疾患を有する患者に関しては抗凝血療法が重要である.
