Management of sphenoid CSF leaks is complicated in the setting of prior surgery and radiation. Active CSF leak in the presence of recalcitrant sinus infections, cranial base osteomyelitis, and life-threatening bleeding offers additional complexity to the hierarchy of treatment strategies.

Postoperative cerebrospinal fluid leak has occurred in sufficient magnitude to require secondary surgical closure in five cases (cases 8, 25, 50, 51, and 53). In several other cases there has been minor spinal fluid leak for several days postoperatively but spontaneous resolution has occurred.

We report on the clinical syndrome of delayed cerebrospinal fluid leak following septoplasty. We describe 2 such cases that we treated, and 1 other case described in the literature. A review of these 3 cases indicates a characteristic clinical and radiologic presentation and a consistent site of cerebrospinal fluid leak.

Cerebrospinal fluid (CSF) rhinorrhea is an uncommon but important medical condition. It can result from trauma, intracranial hypertension, or be idiopathic in origin. If left untreated, significant sequelae can result including infectious meningitis. Beta-2 transferrin is a sensitive and specific protein marker for CSF.
