Abstract The definitive diagnosis of patients with indeterminate biliary strictures remains challenging. Probe‐based confocal laser endomicroscopy (pCLE) provides real‐time histological assessment of bile duct tissues. Since no previous studies have evaluated the efficacy of pCLE under direct cholangioscopic visualization for biliary strictures that ...

Abstract Background Stent migration and sludge formation remain significant problems associated with covered self‐expandable metal stents (CSEMSs). The EGIS biliary stent fully covered flare type (EGIS biliary stent), a new type of polytetrafluoroethylene‐coated self‐expandable metal stent with low axial force and an anti‐migration system, was ...

Perforation of the bile duct caused by endoscopic papillary large balloon dilation: A case report
Abstract The patient was a woman in her 70s with multiple large biliary stones. Lithotripsy was attempted after endoscopic papillary large balloon dilatation. During balloon dilation, inflator resistance, and body movement due to patient pain were observed, and maximum pressure was required for the disappearance of the balloon waist.

Abstract Objectives Malignant double obstruction, defined as the simultaneous presence of biliary and gastric outlet obstruction, represents a challenging clinical scenario. Previous retrospective experiences have demonstrated shorter dysfunction‐free survival (DyFS) of endoscopic ultrasound‐guided choledochoduodenostomy (EUS‐CDS) versus EUS ...

Abstract Objective Limitations are sometimes encountered in the application of laparoscopic cholecystectomy to the treatment of acute cholecystitis. Endoscopic gallbladder stenting (EGBS) has emerged as an additional option. However, the long‐term stent patency remains an issue.

Operative cholangiography was first used by Mirizzi in 1932, but still has not gained general acceptance by surgeons. With these facts in mind, this article reviews 31 cholecystectomies, 20 having been performed by the author, in which operative cholangiography was used, and discusses its technique, difficulties and results.

Abstract Objective Endoscopic sphincterotomy (EST), especially when anticoagulants are used, carries a significant risk of delayed bleeding. However, the relationship between the use of antithrombotic agents, including direct oral anticoagulants, and post‐EST bleeding remains unclear.

Feasibility of endoscopic ultrasound‐guided hepaticogastrostomy using a novel long balloon catheter
Abstract Objectives Recently, a novel long balloon catheter for tract dilation in endoscopic ultrasound‐guided hepaticogastrostomy (EUS‐HGS) was developed. The balloon measures 6 cm in length, which enables one‐step tract dilation of the gastric wall, liver parenchyma, and bile duct wall, contributing to shorter procedure times and reduced bile leakage.
