Cyclospora in Patients with Traveller's Diarrhea

Following the description in 1986 of a new enteropathogen called Cyanobacteria-like body (CLB), several outbreaks caused by this microorganism have been described. Since then several research teams have agreed to place CLB in the genus Cyclospora and to establish the epidemiological, pathogenical and clinical features of the illness.

Purpose of review: The present review focuses on recent epidemiological and diagnostic advances in cystoisosporiasis and cyclosporiasis. Recent findings: Continuous outbreaks of these parasites occur worldwide. Sporadic cases in the United States are travel acquired, but also domestically acquired.

Cyclospora and cyclosporiasis

Most species of Cyclospora (Protozoa: Apicomplexa: Eimeriidae) are parasites of various reptiles and mammals. C. cayetanensis, which probably infects only humans, is transmitted by way of resistant oocysts voided in the faeces and contaminating food or water. Distribution is worldwide, particularly in regions with a low level of hygiene.

A few years ago, a new Coccidian, "Cyclospora sp', has been found in the stools of both immuno-competents and AIDS patients with diarrhoeal illness. The epidemiology, natural history and clinical manifestations of Cyclospora infection have been charted, but its life cycle has not been described yet.
