Hepatobiliary cystadenomas are rare, truly benign cystic epithelial tumors with the potential to become cystadenocarcinoma. The initial symptom is vague, nonspecific abdominal pain, frequently in middle-aged women, but patients may be asymptomatic. Abdominal ultrasonography may be diagnostic.

In the differential diagnosis of liver cysts, biliary cystadenoma is one of the rarest causes. We have reported a typical case, our patient having relatively asymptomatic, insidiously progressive, massive hepatic enlargement. Ultrasonography and computed tomography were the most helpful tests.

The tumor consisted of many small cysts with multiple papillary projections into their cystic spaces. The projections and cyst walls were lined with pseudostratified mucous-cell epithelium.
