Early puberty in girls

Early puberty is not well defined in paediatric endocrinology. This chapter reviews the current insights on definitions, patient groups and treatment modalities in girls with early puberty. It is concluded that there is no clear evidence for a beneficial effect of gonadotrophin releasing hormone agonist (GnRHa) treatment in auxological terms.

Early puberty in Williams syndrome

Pubertal development was evaluated in nine males and 16 females with Williams syndrome (WS). Our results indicate that puberty in WS occurred earlier than in published population controls; specifically, 90% of menstruating females reached menarche and 83% of pubertal males showed Tanner III pubic hair development prior to the age of 12 years.

Abstract A case report of a patient in whom transient elevation of androgens during early puberty, presumably physiologic, resulted in significant clitoral enlargement is presented. ACTH stimulation showed elevated A5 pathway steroids. The findings are consistent with partial 3-β-ol hydroxysteroid dehydrogenase deficiency.

Simplifying and refining the assessment of early puberty

Endogenous sex steroids can be produced in two places—the gonad or adrenal gland. The gonad in females primarily makes oestrogen and in males testosterone. A key physiological hallmark of puberty is the development of progressively rising pulsatile gonadotropin production by the pituitary gland.
