Eosinophilic gastroenteritis (EGE) is a rare disorder characterized by eosinophilic infiltration of the bowel wall with various gastrointestinal manifestations. Till date only 280 cases have been described in the literature. A high index of suspicion, by excluding other causes of peripheral eosinophilia, is a pre requisite for accurate diagnosis.

Trends in Endoscopist Reporting Rates of Eosinophilic Gastrointestinal Diseases in Japan Evaluated by the Japan Endoscopy Database Project

We analyzed 9,940,870 endoscopic procedures from the Japanese Endoscopy Database (2015–2023), identifying 9,669 eosinophilic gastrointestinal disease (EGID) cases. 48才の女性が,水様性下痢便に続く著明な腹水で,近医より紹介された.末梢血白血球分画で好酸球が42%と著増を示し,また,腹水中の好酸球が84%を占めていた.開腹による小腸楔状切除生検にて,粘膜下層から漿膜下層にわたるほぼ全層に,好酸球の浸潤を認めた.これらから,本例はいわゆるMcNabbらの言うtransmural eosinophilic gastroenteritis with ascitesに相当すると思われる.
