Purpose: FAPI-PET is a promising imaging technique for various malignant as well as non-malignant pathologies. In a recent retrospective analysis, we evaluated the diagnostic value of repetitive early FAPI-PET-imaging with FAPI-02, FAPI-46 and FAPI-74 for malignant, inflammatory/reactive and degenerative pathologies.

Integrated Multi‐Time‐Point 18F‐FAPI PET/CT and Immune Histopathology for Predicting Pathological Complete Response in Microsatellite‐Stable Locally Advanced Rectal Cancer Treated With Neoadjuvant Radiotherapy, Chemotherapy, and Immunotherapy [PDF]
Serial multi‐time‐point 18F‐FAPI PET/CT‐derived stromal kinetics and baseline immune‐histopathological features, including iTLS count and B‐cell density, jointly predicted pCR in MSS locally advanced rectal cancer. The resulting four‐variable model achieved a mean nested cross‐validated AUC of 0.874.

18F‑FAPI‑04 PET/CT imaging of fibroblast activating protein reveals myocardial injury beyond cancer staging. [PDF]
Early detection of myocardial injury in cancer patients remains challenging with conventional diagnostic tools. 18F‐FAPI‐04 PET/CT revealed abnormal myocardial uptake associated with cardiovascular risk factors and biomarkers. 18F‐FAPI‐04 PET/CT enables early, noninvasive detection of myocardial injury, providing additional clinical value beyond cancer

Development of FAPI Tetramers to Improve Tumor Uptake and Efficacy of FAPI Radioligand Therapy
Radiolabeled fibroblast activation protein (FAP) inhibitors (FAPIs) have shown promise as cancer diagnostic agents; however, the relatively short tumor retention of FAPIs may limit their application in radioligand therapy. In this paper, we report the design, synthesis, and evaluation of a FAPI tetramer.
