Allergen Immunotherapy and 36‐Month Allergic Respiratory Disease Trajectories: A Target‐Trial

open access: yesImmunity, Inflammation and Disease, Volume 14, Issue 10, October 2026.
Among 2,713 patients with allergic respiratory disease eligible for allergen immunotherapy (AIT), target‐trial compared 1,971 AIT initiators with 742 eligible non‐initiators over 36 months. AIT initiation was associated with less clinically meaningful worsening (46.3% vs 63.6%; RR 0.73), more clinical remission (47.0% vs 18.3%; RR 2.57), more treatment‐

An exploration of the implementation of FeNO testing and a FeNO-guided algorithm in primary care in England

open access: yes
Introduction: Fractional exhaled nitric oxide (FeNO) testing has been implemented in some areas of England, predominantly for asthma diagnosis. This study is nested in an ongoing randomised controlled trial investigating use of FeNO testing and a FeNO ...

Bronchiectasis as the Sentinel Manifestation of ANCA‐Associated Vasculitis: A Case Report and Literature Review

open access: yesRespirology Case Reports, Volume 14, Issue 10, October 2026.
A 74‐year‐old woman was treated for 7 years as culture‐negative bronchiectasis before palpable purpura, haemoptysis, and acute kidney injury revealed MPO‐ANCA vasculitis with pauci‐immune crescentic glomerulonephritis. Rituximab‐based induction produced near‐complete resolution of the pulmonary consolidations and sustained clinical remission at 15 ...

Nasal Epithelial Immune Signatures Are Associated With Age‐Dependent Asthma Trajectories in Early Life

open access: yesAllergy, Volume 81, Issue 10, Page 3533-3553, October 2026.
Nasal epithelial transcriptomics identified three cytokine‐associated epithelial immune programs linked to childhood asthma phenotypes. Age‐associated epithelial polarization shifted from IFN/type‐17‐associated toward type‐2‐associated programs after 3 years of age.

Updates in COPD Management: From Inflammatory Pathways to Targeted Therapies

open access: yesEuropean Journal of Clinical Investigation, Volume 56, Issue 10, October 2026.
COPD is a heterogeneous disease driven by overlapping neutrophilic and eosinophilic inflammatory pathways. This review highlights the shift from symptom‐based treatment toward phenotype‐ and biomarker‐directed care. Blood eosinophils, FeNO, and IgE predict corticosteroid responsiveness and identify candidates for biologic therapy.

Early predictors of persistent childhood asthma and allergic rhinitis: A machine learning study using post‐hoc explainability

open access: yesPediatric Allergy and Immunology, Volume 37, Issue 10, October 2026.
Machine learning models applied to detailed early‐life data showed limited ability to predict which children developed persistent asthma or allergic rhinitis. Although clinically plausible features contributed to predictions, model performance was insufficient for reliable individual prognostic stratification.

FeNO and Exercise Testing in Children at Risk of Asthma

open access: yes, 2018
Background: Exercise testing is the gold standard for diagnosing exercise-induced bronchoconstriction in children, but requires considerable cooperation and medical resources. Therefore, fraction of exhaled nitric oxide (FeNO) has been proposed as a tool
