This report describes the unique clinical course and long‐term survival of two patients with unilateral PPFE who survived for > 10 years. None of the patients had a history of pleurisy or asbestos exposure. However, the certainty of evidence is limited by small sample sizes, methodological heterogeneity, and limited number of available studies.
