Papaverine has offered new options for therapy in erectile dysfunction. Various complications have been reported with papaverine, the prominent ones being priapism and liver function abnormalities. We present a previously unreported case of a fixed drug eruption caused by papaverine.

Solitary fixed drug eruptions involving the lips and mouth are known to occur but are rarely seen in oral medicine clinical practice. Various drugs have been implicated as causative agents. We present a case of a fixed drug eruption on the lip caused by oxybutynin, a relationship not previously described.

Causes of Fixed Drug Eruptions

Fixed drug eruptions (FDE) were investigated in 14 patients. The clinical pattern of the disease was classical. The causes of FDE were different in India as compared to other countries. Sulphonamides and analgesics were the commonest causes, though some other drugs such as tetracycline were also found to be responsible for this syndrome.

Drugs Causing Fixed Drug Eruptions

30 cases of fixed drug eruptions were investigated for their causative drugs. The clinical picture of the cases was classical. The results were confirmed by provocation tests. Analgesics and tetracyclines were found to be the most common offenders. Oxyphenbutazone was the next in the list.

Drug eruptions often have nonspecific clinical findings, and the evaluation of the probability of an eruption being a drug-induced event is difficult. A few types of drug eruption do not present such problems, and the fixed drug eruption is one of those whose clinical findings are specific enough to allow a diagnosis.
