TELANGIECTATIC FOCAL NODULAR HYPERPLASIA CONCOMITANT FOCAL NODULAR HYPERPLASIA
症例は34歳,男性.原発性アルドステロン症に対する左副腎摘出時,偶然肝外側区域の腫瘤を指摘され当科を受診.血液検査上,肝炎ウィルスマーカーは陰性,肝機能にも異常を認めなかった.腹部画像所見では,肝外側区域の腫瘤は約6cm大であり,S3のグリソンが腫瘤内を貫通し,それにより腹側部と背側部,2つの領域に分けられる雪だるま状の形態を呈していた.MRI上両者の造影パターンは異なり,腹側部は中心性瘢痕がみられFNHと診断したが,背側部は腹側に比べ,血管造影CT上早期濃染が強く,悪性疾患も考慮され質的診断には至らなかった.手術は肝外側区域切除を行った.切除標本の割面では,腹側部は中心性瘢痕を伴う黄白色調の病変であり組織学的にFNHと診断した.背側部は黄緑色調であり,組織学的には ...
