Fontan Hemodynamics

Journal of Cardiac Surgery, 1988
Right atrial-pulmonary artery connection places the pulmonary circulation in series with the systemic circulation rather than the single ventricular "parallel" circulatory arrangement that usually is present prior to repair. The accompanying central cardiac shunt and volume overload physiology are eliminated. Favorable changes in ventricular dimension,

The Adult Patient with a Fontan

Cardiology Clinics, 2020
The authors summarize the most important anatomic and physiologic substrates of Fontan circulation. Common anatomic substrates include hypoplastic left heart syndrome, tricuspid atresia, double inlet left ventricle, and unbalanced atrioventricular septal defects. After the Fontan operation exercise capacity is limited and the key hemodynamic drivers is

Compression of an Extracardiac Fontan Following Classic Fontan Revision

Journal of Cardiac Surgery, 2004
The extracardiac Fontan arose as an alternative in order to prevent the classical procedure's sequelea. Complications of the extracardiac Fontan have been described, but this case report aims to highlight the devastating and infrequent complication of external conduit compression.A 25-year-old male underwent Fontan conversion to extracardiac Fontan for

Fontan with pedicled pericardium

The Japanese Journal of Thoracic and Cardiovascular Surgery, 1999
Pedicled pericardium is a useful viable material for cardiac surgery. In an adolescent case, the extracardiac lateral tunnel of a Fontan connection was successfully constructed with pedicled pericardium. This procedure is expected to allow the growth of the tunnel and to need no anti-coagulant therapy, while careful long-term follow-up is necessary.

The hemi-Fontan operation

Pediatric Cardiac Surgery Annual of the Seminars in Thoracic and Cardiovascular Surgery, 2003
In hearts with a functional single ventricle, cavity volume and myocardial muscle mass increase as a consequence of the excessive volume load associated with parallel pulmonary and systemic circulations. The hemi-Fontan operation was conceived as a means of accomplishing early reduction of the volume work of the single ventricle, in anticipation of ...

Management of the Failing Fontan

Seminars in Thoracic and Cardiovascular Surgery: Pediatric Cardiac Surgery Annual, 2015
With and increasing number of early survivors after the palliation of the single ventricle physiology there is a burgeoning Fontan population worldwide that will pose unique challenges because of the inevitable sequelae related to the absence of the alleged "needless" sub-pulmonic ventricle. The increasing number and older-age single-ventricle patients

The Extracardiac Fontan Procedure

Seminars in Thoracic and Cardiovascular Surgery: Pediatric Cardiac Surgery Annual, 1998
The extracardiac modification of the Fontan procedure for complex single ventricle congenital cardiac disease is the latest attempt at improving on both the short-term and long-term results of this operation. This procedure differs from other modifications in that the inferior vena cava is detached from the heart and connected via a conduit to the ...

Fontan Physiology Revisited

Anesthesia & Analgesia, 2015
The Fontan operation places the systemic and pulmonary circulations in series, driven by a single ventricular chamber. It has become the treatment strategy of choice for palliating single-ventricle congenital heart disease. This anatomy engenders profound changes in physiology, affecting the cardiovascular and respiratory systems with direct ...

The adult with a Fontan operation

Current Cardiology Reports, 2007
Since 1971, the Fontan operation has been performed for the repair of single-ventricle physiology. This ingenious operation commits a single ventricle to the systemic circulation and takes advantage of cardiovascular and respiratory physiology to propel deoxygenated blood to the lungs, thus minimizing right-to-left shunting and cyanosis.

Transplantation in the Fontan Patient

Seminars in Thoracic and Cardiovascular Surgery: Pediatric Cardiac Surgery Annual, 2015
The failing Fontan circulation presents difficult treatment challenges. When Fontan revision and or intervention for treatable arrhythmias is not feasible, heart transplantation is the only therapeutic option. Particular challenges presented by these patients include limited ability to assess hemodynamics, complex anatomy, multiple prior procedures ...
