Biomarkers and the Fontan Circulation [PDF]

open access: yesJournal of the American Heart Association, 2016
Although the dramatically improved survival of children with single‐ventricle heart disease with Fontan circulation in the recent era is a tremendous accomplishment in pediatric cardiology and cardiac surgery, it has been accompanied by a growing realization that Fontan circulation is largely ...

Optimal Fenestration of the Fontan Circulation

open access: yesFrontiers in Physiology, 2022
In this paper, we develop a pulsatile compartmental model of the Fontan circulation and use it to explore the effects of a fenestration added to this physiology. A fenestration is a shunt between the systemic and pulmonary veins that is added either at the time of Fontan conversion or at a later time for the treatment of complications.

Cavopulmonary assist for the failing Fontan circulation: impact of ventricular function on mechanical support strategy [PDF]

open access: yes, 2014
Mechanical circulatory support--either ventricular assist device (VAD, left-sided systemic support) or cavopulmonary assist device (CPAD, right-sided support)--has been suggested as treatment for Fontan failure.

End‐Organ Function and Exercise Performance in Patients With Fontan Circulation: What Characterizes the High Performers?

open access: yesJournal of the American Heart Association: Cardiovascular and Cerebrovascular Disease, 2020
Background The physiologic hallmarks of the Fontan circulation—chronically elevated central venous pressures and low cardiac output—have significant effects not only on cardiovascular status but also impact other organ systems.

Outflow boundary conditions for 3D simulations of non-periodic blood flow and pressure fields in deformable arteries [PDF]

open access: yes, 2010
The simulation of blood flow and pressure in arteries requires outflow boundary conditions that incorporate models of downstream domains. We previously described a coupled multidomain method to couple analytical models of the downstream domains with 3D ...

Hypoalbuminaemia predicts outcome in adult patients with congenital heart disease [PDF]

open access: yes, 2015
Background In patients with acquired heart failure, hypoalbuminaemia is associated with increased risk of death. The prevalence of hypoproteinaemia and hypoalbuminaemia and their relation to outcome in adult patients with congenital heart disease (ACHD ...

Fontan Circulation over Time

open access: yesThe American Journal of Cardiology, 2017
The unique, unphysiological Fontan circulation is associated with an impaired functional status of the patients that is suggested to deteriorate over time. Unfortunately, previous studies did not integrate both pulmonary and cardiac determinants of functional status.

Exercise as therapy for neurodevelopmental and cognitive dysfunction in people with a Fontan circulation: A narrative review

open access: yesFrontiers in Pediatrics, 2023
People with a Fontan circulation are at risk of neurodevelopmental delay and disability, and cognitive dysfunction, that has significant implications for academic and occupational attainment, psychosocial functioning, and overall quality of life ...

Anesthetic management of hepatectomy for the patient with Fontan circulation: a case series

open access: yesJA Clinical Reports, 2022
Background Hepatectomy for patients with Fontan circulation consists of high central venous pressure and low pulmonary vascular resistance, and is challenging for physicians.

Exercise Intolerance, Benefits, and Prescription for People Living With a Fontan Circulation: The Fontan Fitness Intervention Trial (F-FIT)—Rationale and Design

open access: yesFrontiers in Pediatrics, 2022
Background: Despite developments in surgical techniques and medical care, people with a Fontan circulation still experience long-term complications; non-invasive therapies to optimize the circulation have not been established.
