We report 3 patients with (sub)acute onset of dyspnea and orthopnea caused by bilateral phrenic neuropathy as a neuralgic amyotrophy (NA) subtype. Excruciating pain, weakness, and atrophy were absent at presentation. All 3 patients were middle-aged men and had serologic evidence of acute hepatitis E virus (HEV) infection (IgM and PCR positive).
