A Longstanding Hungry Bone Syndrome

Hungry bone syndrome is a common sequelae of parathyroid surgery. The phenomenon is caused by severe osteitis fibrosa cystica causing rapid remineralization of the bones. We describe a case of hungry bone syndrome with profound and longstanding hypocalcemia after parathyroid surgery that was very resistant to treatment.

The Hungry Bone Syndrome (HBS) represents an important cause of prolonged hypocalcemia after parathyreoidectomy (PTX) due to primary, secondary or tertiary hyperparathyreoidism. The sudden postoperative withdrawal of parathyroid hormone (PTH) induces a stop in osteoclastic bone resorption without affecting the osteoblastic activity.

In this article, we report the case of a 48-year-old woman with bone pain and multiple brown tumors as the first manifestation of primary hyperparathyroidism due to a large parathyroid adenoma.
