Hypocalcaemia following thyroidectomy for thyrotoxicosis

Temporary postoperative hypocalcaemia is a relatively common complication following thyroidectomy for thyrotoxicosis. Damage, devascularization or inadvertent removal of the parathyroid glands is the most widely accepted reason, but other causes have been postulated including release of calcitonin during operative manipulation and "hungry bone syndrome'

THE EFFECT OF HYPOCALCAEMIA ON THE SECRETION OF CALCITONIN

SUMMARY Hypocalcaemia was produced by i.v. infusion of EDTA into pigs from which the thyroid venous effluent could be quantitatively collected. Calcitonin in the effluent was measured by radioimmunoassay. Reduction of plasma calcium concentration rapidly suppressed the secretion of calcitonin.

1 Fluoride intoxication leads to sudden cardiac death which has been assumed to result from the accompanying severe hypocalcaemia. The aim of this study has been to investigate the suggestion that fluorapatite formation rather than CaF2 precipitation is responsible for this low calcium.

A case of hyperkalaemia associated with hypocalcaemia

An 84-year-old woman presented with lethargy and anorexia. Although routine biochemistry demonstrated mild hyponatraemia, moderate hyperkalaemia and severe hypocalcaemia, the patient did not demonstrate the usual symptoms of hypocalcaemia. An electrocardiogram did not demonstrate evidence of hyperkalaemia or hypocalcaemia.
