When is hypospadias not hypospadias?
Medical Journal of Australia, 1996Hypospadias is a deformity of urethral and penile development in the male. Some cases are secondary to more serious disorders of sexual differentiation. Simple clinical rules can sort the common morphological defect of hypospadias from the rarer but more dangerous diagnosis of intersex.

There are four forms of chordee that do not involve hypospadias: cutaneous chordee, fibrous chordee, corporocavernosal chordee, and congenital short urethra. From 1981 to 1991, 214 patients aged 18 months to 20 years underwent surgical treatment. Among these, 29 individuals developed a crippled penis due to the failure of surgery for the hypospadias ...

When we began our association for the purpose of treating hypospadias in the early 1950s we used a tube graft of full thickness skin in a 1-stage procedure. We have made modifications to improve the appearance of the glans and now use 1 of 4 procedures, dependent on the location of the urethral meatus.

Distal hypospadias, where the urethral meatus is located subcoronal or more distal, is one of the most common congenital anomalies in males. The embryology and etiology of hypospadias, timing of repair, surgical techniques, anesthesia for and complications of hypospadias repair are reviewed.

A careful histological study of chordee tissue obtained in 30 consecutive cases of hypospadias has shown the presence of a considerable amount of smooth muscle. Preputial skin contains some elastic tissue. This feature may well provide significant advantages in the use of tubed preputial skin for urethral repair.
