Abstract Four instances of prolonged nephrographic phase in patients with arterial hypotension are reported. Prolonged bilateral nephrograms during excretory urography should alert the radiologist to the possibility of arterial hypotension. Although the mechanism of the hypotensive nephrogram is not fully understood, continued glomerular filtration ...

Postural hypotension is a common and important problem in elderly patients and necessitates a careful review of medication and lifestyle factors which may precipitate it. Drugs to be avoided if possible include diuretics, alpha-adrenoceptor blocking agents and nitrates.

Summary The development of induced hypotension is discussed and its hazards briefly outlined. The superiority of trimetaphan camphor‐sulphonate (Arfonad) for hypotensive purposes is stressed. The restricted utility of induced hypotension in neurosurgery, and the value of simpler alternatives for the control of bleeding, are indicated.

Intradialytic hypotension (IDH) is common in children during conventional, 4 hour haemodialysis (HD) sessions. The declining blood pressure (BP) was originally believed to be caused by ultrafiltration (UF) and priming of the HD circuit, however emerging data now supports a multifactorial aetiology. Therefore strategies to improve haemodynamic stability

Insulin‐induced hypotension and neurogenic orthostatic hypotension
Neurology, 1986Insulin-induced hypoglycemia induced a fall in blood pressure (BP) in patients with idiopathic orthostatic hypotension (IOH) and multiple system atrophy (MSA), but not in control subjects. Only in IOH was there a correlation between plasma norepinephrine (NE) levels and maintenance of BP during the test. The hypotension was not affected by pretreatment

Postprandial hypotension is both common in geriatric patients and an important but under-recognized cause of syncope. Other populations at risk include those with Parkinson disease and autonomic failure. The mechanism is not clearly understood, but appears to be secondary to a blunted sympathetic response to a meal.

In a retrospective study of necropsies done in a general hospital over five years (1971-75), fatal strokes in elderly patients often appeared to be due to acute hypotension caused by such extracranial events as heart-failure, occult haemorrhage, or multiple pulmonary emboli.

: Supine hypertension commonly occurs in patients with neurogenic orthostatic hypotension due to autonomic failure. Supine hypertension promotes nocturnal sodium excretion and orthostatic hypotension, thus, interfering with quality of life.
