Resistant to Ablation, Responsive to Hydroquinidine: Precision Medicine Guides Recovery in MEPPC Syndrome Cardiomyopathy

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
We report a case of MEPPC syndrome presenting with severe dilated cardiomyopathy due to a pathogenic SCN5A p.Arg814Trp variant. Genetic diagnosis enabled precision pharmacotherapy with hydroquinidine, which suppressed multifocal Purkinje‐origin ectopy resistant to catheter ablation and led to marked improvement in left ventricular function.

Idiopathic ventricular outflow tract arrhythmias from the great cardiac vein: Challenges and risks of catheter ablation

open access: yes, 2013
INTRODUCTION Catheter ablation for idiopathic ventricular arrhythmia is well established but epicardial origin, proximity to coronary arteries, and limited accessibility may complicate ablation from the venous system in particular from the great ...

Idiopathic Outflow Tract Ventricular Arrhythmia Ablation: Pearls and Pitfalls

open access: yesArrhythmia & Electrophysiology Review, 2019
Idiopathic outflow tract ventricular arrhythmias (VAs) occur typically in patients without structural heart disease. They are often symptomatic and can sometimes lead to left ventricular systolic dysfunction. Both activation and pace mapping are utilised for successful ablation of these arrhythmias.

Efficacy of Fixed Low‐Power Long‐Duration Radiofrequency Ablation of Ventricular Arrhythmias Originating From the Left Ventricular Summit

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
Efficacy and safety of a fixed low‐power, long‐duration radiofrequency catheter ablation of ventricular arrhythmias originating from the left ventricular summit. ABSTRACT Background Radiofrequency catheter ablation (RFCA) of ventricular arrhythmias originating from the left ventricular summit (LVS‐VAs) remains challenging.

Late gadolinium enhancement CMR in patients with tachycardia-induced cardiomyopathy caused by idiopathic ventricular arrhythmias

open access: yes, 2012
Background: Idiopathic ventricular arrhythmias in the form of monomorphic premature ventricular contractions (PVC) and/or ventricular tachycardia (VT) can cause tachycardia-induced cardiomyopathy (TICMP).

The “Dead-End Tract” and Its Role in Arrhythmogenesis

open access: yesJournal of Cardiovascular Development and Disease, 2016
Idiopathic outflow tract ventricular arrhythmias (VAs) represent a significant proportion of all VAs. The mechanism is thought to be catecholamine-mediated delayed after depolarizations and triggered activity, although other etiologies should be ...

Left Ventricular Global Longitudinal Strain Is Associated With Appropriate Therapies for Ventricular Arrhythmias Independent of Left Ventricular Ejection Fraction in Patients With Primary Prevention Defibrillators

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
Reduced (absolute value) left ventricular global longitudinal strain is independently associated with an increased risk of appropriate therapies from primary prevention implantable cardioverter defibrillators. ABSTRACT Background Left ventricular ejection fraction (LVEF) has not demonstrated reliability in predicting ventricular arrhythmias or ...

Idiopathic Submitral Left Ventricular Aneurysm: an Unusual Substrate for Ventricular Tachycardia in Caucasians

open access: yes, 2005
Annular submitral aneurysms have been rarely reported in Caucasians. They are typically diagnosed in non-white adults who present with severe mitral regurgitation, heart failure, systemic embolism, ventricular arrhythmias, and sudden cardiac death.

Idiopathic ventricular arrhythmias arising from the left ventricular outflow tract: Tips and tricks

open access: yesJournal of Arrhythmia, 2014
Idiopathic left ventricular outflow tract ventricular arrhythmias (LVOT-VAs) arising from the LVOT are rare compared with the VAs arising from the RVOT. Idiopathic LVOT-VAs have been divided into four subgroups based on successful catheter ablation sites:

Current Topics of Progressive Cardiac Conduction Disease

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
Many genes and the protein cause PCCD. Mutation of NaV1.5 or CX40 cause isolated PCCD, but mutation of lamin A/C, emerin, or desmin lead to cardiomyopathy, and PCCD. Mutation of transcription factor NCX2‐5, and Tbx5 associated with atrial septal defect and abnormal development of conduction system.
